Fees for mental health and substance abuse services may be covered by most managed care insurance plans. Be sure and have your insurance card available when you contact us for services. We will verify your benefit eligibility to determine your deductible, co-payment, plan limits including preexisting conditions and any necessary preauthorization required prior to admission to our agency.
Clients seeking to access Employee Assistance Program (EAP) benefits provided by their employer must disclose this prior to accessing services from our agency. Typically, the client is required to contact the EAP prior to contacting us. The EAP will make a referral to us along with an authorization for the number of sessions allowed by the EAP plan. Failure to notify us in advance that you wish to utilize your EAP benefit may result in unnecessary out-of-pocket expenses.
Clients choosing not to use their insurance or EAP benefit will be asked to provide the total annual household income and number of dependents. From this information, we will determine the discount percentage applied to our usual and customary fees to arrive at an affordable fee for services.
